Obstruction of an Endotracheal Tube due to Aspirated Blood and Secretion in the Postoperative Period

Abstract
In this case, a 26-year-old woman was scheduled for Lefort II and sagittal split ramus osteotomy (SSRO). The patient was intubated with a 6.5 mm nasotracheal RAE (Ring-Adair-Elwyn) tube via nasal route without difficulty and maintained with intubated state for airway maintenance during postoperative period. At postoperative 1 day, patient complained of dyspnea and oxygen desaturation had been developed. Tracheobronchial suction was attempted, but the suction catheter did not go through the endotracheal tube. Endotracheal tube obstruction was detected immediately using flexible fiberoptic bronchoscopy. Therefore, the endotracheal tube was extubated and mask ventilation was applied. Extubated endotracheal tube was obstructed by blood clot. We conclude that aspirated blood and secretion through the fold of cuff caused obstruction of the endotracheal tube.
